Q: What do I do if Social Security has denied my claim for the second time?

A: You may appeal the decision by completing and filing a Request for Hearing By an Administrative Law Judge (Form HA-501-U5) and a Disability Report - Appeal form with the local Social Security field office within 60 days of the denial letter.  You will need to include any new information regarding any medical treatment you may have received.
